Ontario Youth Apprenticeship Program – Skilled Trades Ontario Request for Registration Form

The Ontario Youth Apprenticeship Program (OYAP) Skilled Trades Ontario (STO) Request for Registration Form is now posted on the Employment Ontario Partners’ Gateway (EOPG). There are two forms in each language (English and French) – one to print out for ink signatures and one that allows for electronic signatures.

The Request For Registration Form (No Signature) and the Request For Registration Form (With Signature) are posted on the OYAP Application Process page on the EOPG.

As of April 2, 2025, STO is responsible for registering all training agreements (TAs), including OYAP. OYAP providers must obtain signatures on the Request for Registration Form before creating a TA in the Employment Ontario Information System-Apprenticeship (EOIS-APPR).
